Dog Health & Behavior Disclosure

The purpose of this form is to disclose all known behavioral issues and health conditions of the Dog that may affect training. The Trainer relies on the accuracy and completeness of this information to provide safe and effective services.

Please upload your dog's vaccinations. Rabies and DHPP (or equivalent) are required for boarding and training. Bordatella is required for boarding.

CIV, Leptospiroris, Flea and Tick meds are recommended.

Please also upload a veterinary document dated within the last 6 months showing your dog's weight.
